Home Care Nurse

About the role

We are seeking a compassionate Home Care Nurse to provide safe, high‑quality nursing services to clients in the Woodlands area. You will work independently in clients' homes, delivering clinical care, education, and support to patients and their families.

Key responsibilities

• Deliver safe care and make sound clinical decisions during each home visit.
• Assess and document clients' conditions, updating nursing care plans as needed.
• Perform nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, and medication preparation.
• Maintain accurate nursing documentation and report any incidents or changes in client status.
• Provide training and education to caregivers and family members on home health procedures.
• Participate in quality‑improvement projects and identify service gaps to enhance home care delivery.
• Support integrated home health and palliative care projects, including telesupport services.

Required profile

• Diploma in Nursing.
• Minimum 2 years of experience in community nursing or home care.
• Experience or training in palliative or oncology care is an advantage.
• Ability to perform complex nursing procedures without on‑site supervision.
• Strong independent decision‑making and a positive, empathetic attitude toward client care.

Required skills

• Proficiency in Microsoft Office applications.
