Senior Staff Nurse – Community Mental Health Team

About the role

The Institute of Mental Health is seeking a Senior Staff Nurse to deliver comprehensive psychosocial rehabilitation and medication management for clients with mental illness living in the community. You will work closely with a multidisciplinary team to ensure continuity of holistic care.

Key responsibilities

• Conduct home visits and perform bio‑psycho‑social assessments of clients and their environments.
• Develop, implement and document individualized care plans in collaboration with the multidisciplinary team.
• Administer prescribed medications, including depot injections, and monitor client compliance.
• Identify changes in mental or physical status and promptly inform the doctor.
• Provide counseling, education and psychological support to clients and caregivers.
• Maintain accurate documentation and ensure safe storage of medications.

Required profile

• Passion for community mental health work and strong interpersonal skills.
• Ability to work independently, make sound decisions and build trusting relationships with diverse clients.
• Registered with the Singapore Nursing Board and at least 2 years of acute‑ward experience.
• Registered Mental Health Nurse qualification.
