Palliative Care Staff Nurse – Home Care

About the role

Metta Hospice Care is seeking a compassionate Staff Nurse to join its home‑based palliative care team in the eastern region of Singapore. You will provide high‑touch, person‑centred nursing care to terminally ill patients, supporting both patients and families through the end‑of‑life journey.

Key responsibilities

• Conduct comprehensive home‑based nursing assessments and develop individualized care plans focused on pain relief and symptom management.
• Administer medications and perform procedures such as nasogastric tube (NGT) insertion, catheter care, and venting in line with physician orders.
• Educate and empower caregivers on nursing techniques, medication administration, and recognising end‑of‑life signs.
• Provide psychosocial support and bereavement care to families.
• Act as liaison between patients, families, and the multidisciplinary team, coordinating referrals to inpatient hospices or emergency services as needed.
• Maintain accurate clinical documentation, targeting an average of six home visits per day.

Required profile

• Registered Nurse with a valid Singapore Nursing Board practicing certificate.
• Diploma or degree in Nursing; advanced diploma or graduate certificate in Palliative Care or Gerontology is highly preferred.
• Minimum three years of clinical nursing experience, preferably in home care, community nursing, or oncology.
• Strong clinical judgment, excellent communication skills, and a resilient, patient‑centred mindset.
