Assistant Care Coordinator (NUHS@Home)

About the role

NUHS is looking for an Assistant Care Coordinator for its NUHS@Home service, which delivers home hospital care. You will work closely with healthcare providers, patients and their families to coordinate and manage care in the comfort of patients' own homes. The role is split between patient care (80%) and secondary duties such as equipment coordination and quality improvement (20%).

Key responsibilities

• Perform pre-admission screening for patients and caregivers.
• Work with nurses to manage holistic care at home, monitoring patients' condition and progress.
• Report deviations from the norm, care gaps or issues to the Team Nurse Lead, nurses or clinicians.
• Handle enquiries and give regular updates on care plans to patients and caregivers.
• Take part in interdisciplinary team meetings and contribute to care plans.
• Document care and keep accurate records of progress and treatment plans.
• Collaborate with community resources and other providers for continuity of care and referrals.
• Develop care plans based on assessed care needs.
• Monitor patients' vital signs via the telehealth system and escalate red flags to team nurses or physicians.
• Provide basic patient education and lifestyle advice such as diet, nutrition and basic exercise.
• Ensure equipment and supplies are available and coordinate with vendors; take part in quality improvement and risk management activities.

Required profile

• Diploma in a health-related course preferred.
• Fluent English; speaking local dialects is an advantage.
• Able to work shifts, including evenings, weekends and public holidays.
• Comfortable working independently in a fast-paced environment.
• Service-oriented, with strong interpersonal and communication skills, teamwork and initiative.
• Previous healthcare experience or knowledge of hospital operations is an added advantage.

Required skills

• Good computer literacy and knowledge of PC software packages.
• Ability to use the telehealth monitoring system.
