Community Care Executive, Senior Day Care

About the role

NTUC Health is looking for a Community Care Executive for its Senior Day Care Centre. You will join an integrated care team delivering a person-centred care model for seniors, including those with dementia, helping to promote their independence and maintain their cognitive and physical function. You will assist the Community Care Manager in keeping the centre running smoothly.

Key responsibilities

• Care delivery: supervise and support the direct care team on Assisted Daily Living (ADL) tasks, coordinate and implement each senior's individual care plan, and keep the environment safe and clean
• Engage regularly with seniors to monitor their cognition and functional well-being, and collaborate with healthcare professionals to track and report concerns
• Run centre-based programmes such as intergenerational and volunteer programmes, tailor them to guidelines and seek feedback to improve them
• Prepare documentation to support incident reporting and support next-of-kin and caregivers with information and guidance on care
• Maintain centre amenities, arrange client transport, monitor inventory and liaise with vendors, technicians and facilities management
• Help trial and evaluate new equipment or technology, gather user feedback, and supervise staff in following crisis response SOPs
• Plan and manage staff rosters and day-to-day changes
• Support the Community Care Manager with audit documents and new non-clinical SOPs, and supervise volunteers, including scheduling and assessing their training

Required profile

• At least a diploma, preferably in social service or a related discipline
• Experience managing and leading teams is an added advantage
• Strong verbal and written interpersonal and communication skills
• Able to work independently and as a team player across all levels
• Growth mindset and willingness to keep learning
• Nice to have: hospitality or community support experience
