About the role

As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as a Pharmacist. The role involves reviewing transfer medications, counselling patients and supporting medication reconciliation.

Key responsibilities

Review transfer medications appropriately and contact patients before transfer about medication supply needs
Answer medicine-related questions and give medication counselling on transfer medications, in person and virtually
Carry out medication reconciliation and review discharge prescriptions
Verify new physician orders and flag inappropriate medications or doses where relevant
Assist with medication supply duties and follow up on medication preparation status
Perform other duties assigned by the NUHS staff member depending on requirements

Required profile

Bachelor of Science in Pharmacy from an accredited college by the Singapore Pharmacy Council
At least 2 years of relevant experience
Fully registered with the Singapore Pharmacy Council
Able to work independently and in a team
Weekdays and/or weekends availability
