Resident Physician (Home Healthcare)

About the role

The Resident Physician joins a Home Healthcare team to provide comprehensive medical care for clients in their homes and via telehealth. You will manage acute and chronic conditions, develop individualized care plans, and support the functional well‑being of elderly patients.

Key responsibilities

• Deliver medical consultations through regular home visits and telehealth sessions.
• Collaborate with a multidisciplinary team to create holistic care plans.
• Engage clients and their families to align care with preferences and goals.
• Perform bedside procedures such as venipuncture, urinary catheterisation, and management of simple tubes and drains.
• Titrate and prescribe medications as needed.
• Coordinate referrals to internal services (home nursing, therapy) and external social services.
• Maintain accurate medical records and produce reports, memos, and referrals.
• Participate in CME sessions, workshops, and conferences.
• Provide teaching to Family Medicine residents when required.

Required profile

• Basic medical degree or postgraduate qualification registrable with the Singapore Medical Council.
• Full registration with the Singapore Medical Council and active clinical practice.
• Relevant experience in home care services is an advantage.
• Ability to work independently and as a team player.
• Strong interpersonal and communication skills.
• Willingness to travel to clients’ homes.
