Senior Pharmacist – Clinical Pharmacy Role

About the role

You will provide pharmaceutical care by working collaboratively with a multidisciplinary health‑care team to promote wellness, maintain health, and intervene in acute or chronic illness.

Key responsibilities

• Provide pharmaceutical care by working collaboratively with a multidisciplinary health‑care team to promote wellness, maintain health, and intervene in acute or chronic illness.
• Recommend and monitor patient drug therapy and provide drug information.
• Identify, resolve and prevent drug‑related problems.
• Interact with patients, families and other health‑care providers to ensure continuity of services and achieve desired patient outcomes.
• Ensure accurate and timely distribution of pharmaceuticals.
• Provide education to patients, staff and other health‑care practitioners.

Required profile

• Degree/Master graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council.
• Full registration with the Singapore Pharmacy Council (pre‑reg to full welcome).
• Pre‑Reg pharmacists turning full registration are welcomed to apply.
• Valid membership with a local professional body/association preferred.
• Experience as a pharmacist in relevant clinical settings and demonstrated competency is an advantage.
