Senior Staff Nurse – Home Care (NUHS@Home)

About the role

The Senior Staff Nurse I/II provides acute medical care to patients in their own homes, collaborating with physicians and multidisciplinary teams to develop and implement individualized care plans. This role involves assessing patients, delivering treatments, and educating families on self‑care and disease management.

Key responsibilities

• Conduct comprehensive patient assessments and create personalized care plans.
• Administer medications, treatments and procedures as prescribed.
• Monitor patient progress, adjust care plans, and document care in electronic medical records.
• Communicate effectively with physicians, healthcare professionals, patients and families.
• Educate patients and families on self‑care techniques and disease management.
• Ensure patient safety, infection control and adherence to ethical and legal nursing standards.
• Participate in quality improvement and research activities.
• Provide clinical supervision and precept junior nurses, contributing to training programs.

Required profile

• Diploma or Degree in Nursing, registered with the Singapore Nursing Board.
• Registered Nurse with 3–5 years post‑registration clinical experience.
• Ability to work independently and as part of a multidisciplinary team.
• Strong organisational, coordination, interpersonal and communication skills.
• Willingness to work flexible schedules, including evenings, weekends and public holidays.

Required skills

• Proficiency in using electronic medical record systems.
