Senior Staff Nurse I/II - NUHS@Home

About the role

The NUHS@Home nurse provides nursing care to patients who need acute medical treatment but can be cared for at home instead of in a hospital. The nurse works with physicians and other healthcare professionals to build and run care plans, track patient progress and teach patients and families self-care. The nurse also gives clinical supervision, mentors junior nurses and helps with NUHS@Home nursing training.

Key responsibilities

• Conduct patient assessments and develop individualised care plans
• Administer medications, treatments and procedures as ordered by the physician
• Monitor and evaluate patient progress and adjust care plans as needed
• Communicate with physicians, colleagues, patients and families about patient care
• Educate patients and families on self-care and disease management
• Maintain patient safety and infection control measures
• Document care in electronic medical records and take part in quality improvement
• Work flexibly, including evenings, weekends and public holidays

Required profile

• Diploma or degree in Nursing, registered with the Singapore Nursing Board
• Registered Nurse with at least 3-5 years of post-registration clinical experience
• Able to work independently and as part of a team
• Strong organisational, coordination, interpersonal and communication skills
• Willingness to take on extra duties assigned by Nursing Officers, Senior Nursing Officers, Assistant/Deputy Director (Nursing) and Chief Nurse
