Senior/Staff Nurse, Community Care

About the role

NUHS Population Health & Community Care (PHCC) is recruiting a Senior/Staff Nurse for its Community Care programme in western Singapore. The nurse conducts care assessments and plans and manages residents' care according to established protocols and evidence-based practices, under appropriate supervision. The role involves coordinating care across the healthcare continuum and working with interdisciplinary teams and community partners.

Key responsibilities

• Provide nursing care and health coaching in health promotion and illness prevention through patient education
• Deliver primary care, including basic assessments and health education to the general public
• Provide home care services for patients moving from hospital to home
• Contribute to evidence-based practice, innovation, quality improvement and/or research projects
• Perform other community-related duties as assigned

Required profile

• Diploma or degree in Nursing and valid registration with the Singapore Nursing Board
• Minimum of two to three years of relevant work experience in a community setting
• Ability to work independently and collaboratively as part of a team
• Strong organisational, coordination, interpersonal and communication skills
• Ability to converse in local languages is preferred
