About the role
We are seeking a Social Worker / Resident Caseworker to provide comprehensive case management and support for residents across Singapore. You will work closely with a multidisciplinary team to enhance the quality of life for approximately 50 residents.
Key responsibilities
- Develop individualized care plans based on residents’ needs and conditions.
- Implement care plans, monitor progress and evaluate care outcomes.
- Develop and coordinate programmes and activities that support residents’ well‑being.
- Prepare six‑monthly care review reports and conduct monthly interviews for around 50 residents.
Required profile
- Degree or Graduate Diploma in Social Work, Psychology or Counselling.
- Willingness to adhere to established Standard Operating Procedures.
- Willingness to work rotating shifts as per the duty roster.
- Minimum of 1 year relevant work experience.
