Assistant Nurse Manager – Home Personal Care

About the role

The Assistant Nurse Manager (HPC & HPC+) supports the Nurse Manager in delivering clinical leadership, governance and operational oversight for Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services. This role blends hands‑on community nursing practice with team supervision and service coordination to ensure safe, ethical and person‑centred care for clients with complex functional, medical and end‑of‑life needs.

Key responsibilities

• Provide direct nursing care and clinical oversight for clients receiving HPC and HPC+ services, including complex and end‑of‑life care.
• Support care planning, clinical decision‑making and escalation in line with organisational protocols.
• Monitor client outcomes, ensure safety, quality and continuity of care, and maintain accurate clinical documentation.
• Assist the Nurse Manager in supervising nursing staff, offering guidance on practice, workload management and standards compliance.
• Coach, mentor and train nurses and care staff, and conduct competency assessments and onboarding.
• Collaborate with multidisciplinary teams, community partners and service providers to deliver integrated care and reduce avoidable hospital admissions.
• Participate in quality improvement, audit and risk management activities, including incident reporting.

Required profile

• Registered Nurse with current registration and eligibility to practice.
• Experience in community nursing, home care or similar settings, preferably with complex or end‑of‑life care.
• Demonstrated ability to supervise and mentor nursing staff.
• Strong clinical judgment, decision‑making and escalation skills.
• Excellent communication and teamwork abilities.
