Care Facilitator – Dementia Day Care Centre

About the role

The Care Facilitator will support seniors in a dementia day‑care centre, assisting the Senior Care Facilitator with daily operations and client wellbeing. This role involves direct interaction with older adults, ensuring their safety, health monitoring, and participation in therapeutic activities.

Key responsibilities

• Assist the Senior Care Facilitator in assessing, planning, implementing and evaluating care for clients.
• Conduct daily vital‑sign checks and provide basic first‑aid for minor injuries.
• Support the Operations Manager in planning and delivering centre and outdoor activities.
• Engage clients in programmes, accompany them to and from the centre, and supervise outdoor outings.
• Respond promptly to emergencies and attend to clients’ immediate needs.
• Assist with activities of daily living for day‑care clients.
• Maintain general cleanliness and follow infection‑control procedures.

Required profile

• Minimum secondary‑school education.
• Preferably holds healthcare or community‑care training and a First Aid Certificate.
• Preferably 2 years of experience in the healthcare sector, though fresh candidates are welcome.
• Ability to communicate in Mandarin, Malay or Chinese dialects is an advantage.
• Team‑oriented with a genuine passion for serving older adults.
