Care Specialist

About the role

Homage is seeking a Care Specialist to deliver high‑quality nursing care and patient education for individuals enrolled in its growth hormone injection programme. The role combines clinic and home visits with remote support, ensuring patients receive compassionate, professional guidance throughout their therapy.

Key responsibilities

• Act as the primary point of contact for patients in the growth hormone injection programme.
• Conduct initial and follow‑up clinic or home visits to teach injection techniques, disease management and therapy adherence.
• Monitor adherence and dose history using required devices, providing re‑training when needed.
• Troubleshoot device issues, arrange replacements and manage consumable replenishment.
• Handle patient enquiries via hotline, phone and WhatsApp with empathy and professionalism.
• Identify and manage adverse reactions in line with standard operating procedures.
• Prepare regular and ad‑hoc reports for internal and external stakeholders.
• Support training of anchor nurses to ensure continuity of care.

Required profile

• Diploma or Degree in Nursing recognised by the Singapore Nursing Board (SNB).
• Full SNB registration with a valid BCLS and AED certification.
• Minimum three years of nursing experience, preferably in a fast‑paced environment.
• Strong initiative, self‑management and ability to work independently.
• Exceptional empathy, patience and a gentle clinical approach, especially with parents and children.
• Excellent communication and teaching skills.
• Willingness to travel for clinic and home visits.
