Case Management Officer – Renal Project (Contract)

About the role

The Case Management Officer will support Project Renalssance, an initiative that demonstrates the effectiveness and sustainability of early palliative‑care options for patients with advanced chronic kidney disease. The role involves coordinating patient appointments, managing referrals to community partners, and delivering education and counseling in line with established clinical pathways.

Key responsibilities

• Coordinate appointments for the Kidney Supportive Care Clinic and arrange referrals to home hospices and integrated home‑health services.
• Provide patient care, including education and counseling, following the approved care plan and clinical pathways.
• Contribute to multidisciplinary patient management and collaborate with physicians and other health‑care providers.
• Apply knowledge of medico‑legal prevention related to patient care.
• Demonstrate proper use, care, and economical consumption of medical equipment and supplies.

Required profile

• Registered Nurse with a minimum of three years of clinical experience.
• Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science; a Renal Nursing certificate/diploma is advantageous.
• Strong clinical skills, critical‑thinking ability, and good interpersonal and communication skills.
• Effective bilingual proficiency.
