Formulary Pharmacist – Hospital Formulary Management

About the role
The Formulary Pharmacist will lead the development, implementation and maintenance of the hospital's formulary system, ensuring optimal medication management, cost‑effectiveness and patient safety across Tengah General and Community Hospital.
Key responsibilities
- Draft and maintain medication management policies, formulary governance guidelines and gate‑keeping processes for new drug requests.
- Collaborate with clinical heads (cardiology, endocrinology, infectious diseases, geriatrics) to align formulary decisions with service needs.
- Conduct benchmarking studies of formulary structures and drug utilisation patterns against other hospitals.
- Lead Antimicrobial Stewardship Program (ASP) initiatives and serve as Value‑Driven Outcomes (VDO) lead.
- Establish and monitor adverse drug reaction (ADR) reporting workflows and medication error monitoring systems.
- Support therapeutic VDO programmes for diabetes, heart failure and immunologic clinics.
- Provide drug information services, handle enquiries, prepare reports and propose evidence‑based alternatives during stock shortages.
- Maintain pharmacy intranet pages, publish ADR and pharmacy bulletins, and manage drug information resources.
- Support the Pharmacy and Therapeutics (P&T) Committee by preparing agendas, drug reviews, minutes and updating the hospital drug list.
Required profile
- Registered pharmacist with relevant clinical pharmacy experience.
- Demonstrated experience in formulary management, antimicrobial stewardship and medication safety initiatives.
- Strong analytical ability to conduct benchmarking and evaluate drug utilisation data.
- Excellent communication skills for stakeholder engagement and committee work.
