Pharmacist – Clinical Pharmacy Position

About the role

IHH Healthcare Singapore is seeking dedicated Pharmacists to join its multidisciplinary clinical teams. You will play a pivotal role in ensuring safe medication use, providing expert drug information, and supporting patient‑centred care across our hospitals.

Key responsibilities

• Review medication orders for appropriateness and dispense accurately.
• Provide medication counselling to patients, including specialised advice on warfarin, inhaler technique and other therapies.
• Deliver drug information and clinical advice to doctors, nurses and patients.
• Supervise and train pharmacy assistants.
• Conduct quality‑assurance activities such as ward inspections and expiry checks.
• Manage controlled‑drug supply, adverse‑drug‑reaction reporting and drug‑usage monitoring.
• Participate in medication reconciliation rounds and contribute to specialised care areas (ICU, transplant, pain management, diabetes).
• Take part in projects and continuous‑improvement initiatives.

Required profile

• Degree or Master in Pharmacy.
• Registered or eligible for registration with the Singapore Pharmacy Council.
• Strong communication and interpersonal abilities.
• Team‑player with a passion for patient‑centred care.
• Willingness to work rotating shifts.
