Pharmacist – Clinical Pharmacy Services

About the role

The Pharmacy team is seeking a qualified Pharmacist to provide operational leadership and clinical expertise. You will ensure medication safety, efficiency and regulatory compliance while supporting patient‑centred care across the hospital.

Key responsibilities

• Provide operational support and lead pharmacy services to maintain medication safety and compliance.
• Deliver specialised pharmacotherapy counselling, focusing on chronic disease management and patient quality of life.
• Coordinate daily workflow to guarantee timely, accurate, and safe medication supply.
• Review and validate prescriptions for legality, completeness, accuracy and clinical appropriateness.
• Resolve medication‑related issues with prescribers and escalate when necessary.
• Perform dispensing, labelling, repackaging and compounding in line with regulatory standards.
• Conduct medication reconciliation during transitions of care and document discrepancies.
• Support discharge processes, adherence programmes and post‑discharge issue resolution.
• Assist with inventory management, stock control, expiry monitoring and procurement activities.
• Utilise clinical systems for documentation, reporting and troubleshooting.
• Provide training, mentorship and competency development for staff and trainees.
• Participate in quality improvement, audits, research initiatives and risk management.

Required profile

• Degree in Pharmacy and registration with the Singapore Pharmacist Board with a valid practising licence.
• Minimum 2 years of relevant experience in hospital, clinical or retail pharmacy settings.
• Strong clinical knowledge of pharmacotherapy and a commitment to lifelong learning.
• Excellent organisational, analytical and decision‑making abilities.
• Effective communication, interpersonal and problem‑solving skills.
• Ability to supervise, guide and mentor junior staff and trainees.
