Resident Physician – Home Care

About the role

Raffles Medical Group is expanding its home‑care service and seeks a Resident Physician to provide comprehensive, continuity‑based care to homebound patients in Singapore. You will act as the named doctor for a panel of elderly and complex‑needs patients, delivering medical visits in the comfort of their homes.

Key responsibilities

• Conduct up to eight home medical visits per day within a structured weekday roster, with one‑hour appointment slots and geographically clustered routes.
• Manage a named patient panel, ensuring continuity of care and regular follow‑up.
• Perform medication reviews, manage chronic conditions and coordinate individualized care plans.
• Collaborate with community nurses, caregivers and other health professionals to deliver integrated care.
• Provide teleconsultations during office hours when required.
• Offer guidance and support to patients’ families and caregivers throughout the care journey.
• Work with an established community care provider without night, weekend or on‑call duties.

Required profile

• Full registration with the Singapore Medical Council.
• Experience or genuine interest in community care, geriatric medicine or palliative care.
• Relevant postgraduate qualifications such as GDFM, MCFM or FCFP.
