Senior Pharmacist – Clinical & Operational Leader

About the role

The Senior Pharmacist will provide clinical and operational leadership to ensure safe, efficient, and compliant pharmacy services. This role combines direct patient care with strategic oversight of pharmacy operations in a hospital setting.

Key responsibilities

• Lead clinical pharmacy services, ensuring high standards of medication safety and patient outcomes.
• Supervise, mentor and coach pharmacy staff in dispensing, medication reconciliation, discharge planning and counselling.
• Optimise pharmaceutical care through medication review, reconciliation and evidence‑based recommendations for chronic and complex conditions.
• Investigate and resolve medication‑related incidents, discrepancies and supply issues, implementing corrective actions.
• Oversee inventory, procurement and medication use processes to achieve cost‑effectiveness and regulatory compliance.
• Drive quality‑assurance initiatives, audits, data analysis and workflow improvements.
• Support clinical information systems, staff training and multidisciplinary collaboration.
• Promote risk‑management practices to enhance patient safety.

Required profile

• Degree in Pharmacy and registration with the Singapore Pharmacist Board with a valid practising licence.
• Minimum 5 years of relevant experience in clinical, operational or leadership roles within a hospital or healthcare setting.
• Strong clinical and pharmacotherapeutic knowledge, especially for complex and chronic conditions.
• Excellent communication, interpersonal and problem‑solving abilities.
• Demonstrated supervisory, coaching and mentoring capabilities.
• Commitment to continuous learning and quality improvement.
