Senior Staff Nurse – Home Care

About the role

The Senior Staff Nurse – Home Care provides patient‑centred, compassionate nursing services to individuals receiving care in their own homes. Reporting to the Nurse Manager (Home Care), you will ensure safe, competent and individualized care that meets organisational standards and regulatory requirements.

Key responsibilities

• Deliver nursing care that is safe, competent and tailored to each patient, including medication administration and wound dressing.
• Assess, monitor and reassess patients’ conditions, develop and adjust care plans, and support families coping with advanced disease.
• Coordinate care with physicians, act as liaison between families and medical team, and participate in multidisciplinary meetings.
• Identify home equipment needs, arrange requisition and allocation of aids, and ensure continuity of care through follow‑up.
• Maintain accurate documentation, uphold infection‑control and drug‑storage standards, and protect patient confidentiality.
• Adhere to Singapore Nursing Board standards, organisational policies and the Code of Ethics, and stay updated on industry guidelines.

Required profile

• Diploma in Nursing from a local polytechnic and registration with the Singapore Nursing Board.
• Post‑registration experience in a hospital or hospice setting is preferred.
• Strong interpersonal and communication skills, meticulous attention to detail, and ability to work under pressure.
• Proficiency with basic IT systems and a commitment to delivering excellent customer service.
• Team‑player attitude and willingness to contribute to a multidisciplinary environment.
