Social Worker

About the role

Join Jamiyah Singapore’s support team to provide compassionate social work services grounded in Islamic values. You will assess client needs, coordinate with government agencies, and deliver counseling and community resources.

Key responsibilities

• Assess clients' psychosocial and behavioral concerns prior to admissions.
• Collaborate with AIC, MOH, and other agencies to ensure continuity of care.
• Provide counseling and connect clients with community resources.
• Evaluate residents' financial obligations and assist with insurance coverage.
• Facilitate ACP and PPC discussions for residents and families.
• Conduct home visits to resolve financial‑social issues.
• Maintain accurate documentation and resident records.

Required profile

• Minimum degree in Social Work or related discipline.
• Registered Social Worker with SASW.
• At least 1 year of relevant experience, preferably in healthcare.
• Strong interpersonal and critical‑thinking abilities.
• Team‑player attitude.
• ACP and InterRai training advantageous.
