Staff Nurse I (NUHS@Home)

About the role

The NUHS@Home nurse provides nursing care to patients who need acute medical treatment but can be cared for at home instead of in a hospital. The nurse works with physicians and other healthcare professionals to build and run care plans, monitor progress and teach patients and families self-care. The nurse also supervises junior nurses, contributes to nursing training and acts as Nurse in-charge.

Key responsibilities

• Assess patients' needs on admission and daily, plan and implement care, and evaluate and monitor progress
• Document care and outcomes on established charts and forms, and carry out treatments and investigations prescribed by physicians
• Communicate patients' progress with physicians and colleagues, follow up on continuity of care and take part in physicians' rounds
• Prepare and assist physicians with procedures and provide patient and family education on disease prevention, health and rehabilitation
• Use informatics and technology to manage productivity and resources, and maintain a clean, safe environment
• Supervise and guide junior staff and nursing students, and assume the role of Nurse in-charge
• Take part in quality activities, research studies and improvements to care and processes
• Accept additional duties assigned by Nursing Officers, Senior Nursing Officers, Assistant/Deputy Director (Nursing) and Chief Nurse

Required profile

• Diploma or degree in Nursing, registered with the Singapore Nursing Board
• At least 1 year of post-registration clinical experience as Staff Nurse II
• Ability to work independently and as a team, with strong organisational, coordination, interpersonal and communication skills
• Flexible schedule, including evenings, weekends and public holidays
