Transplant Coordinator – Organ Donation & Transplant Services

About the role

The Transplant Coordinator will play a pivotal role in supporting organ donation and transplant programmes across SingHealth hospitals. Working within a multidisciplinary team, you will guide patients and families through the transplant journey, from donor identification to post‑transplant care.

Key responsibilities

• Identify potential donors from brain‑death and cardiac‑death pools in SingHealth facilities.
• Support the Organ Donation Oversight Committee and promote organ donation awareness among internal and external stakeholders.
• Order and interpret basic diagnostic and laboratory tests related to transplantation.
• Provide on‑call coverage on a rotational basis for donation and transplant services.
• Deliver coordinated care across multiple transplant programmes, ensuring follow‑up, education and continuity of care pre‑ and post‑transplant.
• Participate in transplant‑related research and quality‑improvement projects.

Required profile

• Degree in Nursing, Psychology, or Life/Health Sciences.
• Strong interpersonal skills for handling sensitive situations.
• Independent work style with a pleasant disposition.
• Willingness to perform rotational on‑call duties and to learn continuously.
• Ability to multitask and meet time‑sensitive deadlines within a multidisciplinary environment.
• Effective team player capable of collaborating with multiple stakeholders.
