Medical Social Worker (Palliative Care)

About the role

The Medical Social Worker in Palliative Care provides emotional, social and practical support to patients with life‑limiting illnesses and their families. The role involves holistic needs assessment, case management, counselling and home‑based outreach to enhance patient well‑being.

Key responsibilities

• Deliver comprehensive psychosocial support, including counselling and practical assistance, to patients and caregivers.
• Conduct holistic needs assessments and develop individualized care plans.
• Coordinate with multidisciplinary teams to integrate social work interventions into overall patient care.
• Maintain active collaboration with external social service agencies, healthcare institutions and relevant community partners.
• Perform home visits and outreach activities to ensure continuity of support.

Required profile

• Degree in Social Work or an equivalent qualification.
• Full registration with the Singapore Association of Social Workers.
• Minimum three years of experience in a hospital, clinical or healthcare setting.
• Strong commitment to patient‑centred care and community support.
• Adaptable, resilient and comfortable working in diverse and dynamic environments.
