Resident Physician, Home Care

About the role

Raffles Medical Group is expanding its home care service and seeks a Resident Physician to provide longitudinal, relationship‑based family medicine to homebound patients. You will act as the named physician for a panel of elderly and complex‑needs patients, delivering personalised care in the comfort of their homes.

Key responsibilities

• Conduct home medical visits on a structured weekday roster, seeing approximately eight patients per day with one‑hour appointment slots.
• Manage a named patient panel to ensure continuity of care.
• Collaborate with an established community care provider and community nurses, caregivers, and other healthcare professionals.
• Provide teleconsultations during office hours when required.
• Oversee chronic condition management, medication reviews, and ongoing care plans.
• Offer guidance and support to patients’ families and caregivers.
• Work without night, weekend, or on‑call duties.

Required profile

• Full registration with the Singapore Medical Council.
• Experience or genuine interest in community care, geriatric medicine, or palliative care.
• Relevant qualifications such as GDFM, MCFM or FCFP.
