About the role
The Institute of Mental Health (IMH) in Singapore, a 2,000‑bed psychiatric hospital located in Buangkok Green Medical Park, is seeking a dedicated Case Manager. The role provides essential case management services to individuals facing mental health challenges, including assessment, treatment planning, and ongoing support within a multidisciplinary environment.
Key responsibilities
- Perform assessments to determine client needs, health status, and social conditions.
- Develop individualized treatment plans, coordinate services, and provide ongoing support to facilitate client recovery.
- Collaborate with clients, families, and multidisciplinary teams to develop intervention plans.
- Respond to client crises by conducting risk assessments, safety planning, and arranging immediate care to stabilize situations.
- Contribute to quality management and general departmental activities.
Required profile
- Diploma or degree holder.
- Background in nursing or allied health with psychiatric clinical experience.
