Medical Social Worker – Homecare

About the role

We are seeking a compassionate Medical Social Worker to provide homecare support to patients and their families. The role involves delivering psycho‑social and emotional care, coordinating services, and supporting end‑of‑life planning.

Key responsibilities

• Explore and provide psycho‑social/emotional care and support to patients and caregivers/family.
• Lead Advance Care Planning (ACP) discussions and complete Preferred Care Plans.
• Provide bereavement support.
• Coordinate in‑house financial assistance for needy patients.
• Arrange medical escort and transport for patients.
• Refer patients for respite care, nursing home placement and other necessities.
• Offer counselling and reconciliation services.
• Conduct crisis intervention.
• Deliver education, teaching and networking activities.

Required profile

• Degree or postgraduate diploma in Social Work.
• Preferably at least 2 years of relevant experience.
• Interest in working with the elderly.
• Compassionate, positive and caring attitude toward patients and clients.
• Excellent communication, interpersonal and problem‑solving skills.
• Meticulous, diligent and a good team player.
• Adaptable and comfortable with change.
