Senior/Staff Community Nurse

About the role

We are seeking a Senior/Staff Nurse to deliver community nursing services as part of an interdisciplinary care team. The role involves providing holistic care to clients in the community, documenting assessments, care plans, interventions and outcomes, and escalating cases as needed.

Key responsibilities

• Collaborate with the interdisciplinary care team and community partners to deliver holistic care.
• Conduct patient assessments and develop or update individualized care plans.
• Document assessments, interventions, and evaluations accurately and promptly.
• Refer or escalate patients to appropriate healthcare providers according to established protocols.
• Ensure compliance with community nursing guidelines and standards.

Required profile

• Diploma or Degree in Nursing.
• Minimum of 2 years relevant community nursing experience.
• Registered with the Singapore Nursing Board.
• Strong interpersonal and communication abilities.
• Ability to work independently and as part of a team.
• Capability to manage multiple tasks in a dynamic environment.

Required skills

• Proficiency in Microsoft Office applications.
