Pharmacist – Clinical Pharmacy Services

About the role

The Pharmacy team is seeking a qualified Pharmacist to lead operational support and ensure medication safety, efficiency, and regulatory compliance. You will provide specialised pharmacotherapy counselling, manage daily workflow, and support discharge processes to improve patient outcomes.

Key responsibilities

• Provide operational leadership within pharmacy services, ensuring medication safety and regulatory compliance.
• Deliver specialised pharmacotherapy counselling focused on chronic disease management and patient quality of life.
• Coordinate daily workflow to guarantee timely, accurate, and safe medication supply.
• Review and validate prescriptions for legality, completeness, accuracy, and clinical appropriateness.
• Perform medication reconciliation across transitions of care and resolve discrepancies.
• Manage inventory, including stock control, expiry monitoring, and discrepancy investigation.
• Support procurement activities, vendor coordination, purchasing, and cost management.
• Utilise clinical documentation systems for reporting and troubleshooting.
• Provide training, mentorship, and competency development for staff and trainees.
• Participate in quality improvement, audits, and research initiatives.

Required profile

• Degree in Pharmacy.
• Registered with the Singapore Pharmacist Board and holding a valid practising licence.
• At least 2 years of relevant experience in hospital, clinical, or retail pharmacy settings.
• Strong clinical knowledge of pharmacotherapy and a commitment to continuous professional development.
• Excellent analytical, decision‑making, and communication skills.
